BEFORE Beginning, you MUST open a NEW gmail account to use when filling out the form.

If you are within the United States, you MUST use only the Vanilla prepaid gift cards, not personal credit cards so you will need to go get them from a store that sells them and YES, each one must be loaded with $40 U.S. Dollars on them (if you are unsure, use this currency converter here).
